Primary Health CareCare Plus Care Plus provides additional funding to PHOs for people who have to visit a GP or nurse more frequently because of a chronic condition, such as diabetes or heart disease, acute medical or mental health needs, or a terminal illness. Overall funding allows for five percent of the New Zealand population to be Care Plus patients. The number of eligible Care Plus patients in each PHO varies according to age, gender, ethnicity and socio-economic status of the enrolled population of the PHO, and the number of the PHOs patients that hold a High Use Health Card. Funding is based on the number of eligible Care Plus patients less the actual number of High Use Health Card patients. The level of funding paid to a PHO for Care Plus services, is dependent on the percentage of the eligible number of Care Plus patients that are actually enrolled in the Care Plus programme.
