Health Promotion


PHOs are further funded to provide appropriate health promotion activities within their communities. PHOs are required to meet certain reporting criteria to be eligible for Health Promotion capitation based funding.

Health promotion funding is calculated based on $2.02 per enrollee with different multipliers for differing groups of need. The multipliers applied are:

Note: This capitation funding is not available to those PHOs that have not signed up to the Health Promotion initiative or applicable to enrolees who have a High User Health Card.
