Project / Service DescriptionThis initiative is aimed at supporting the prospective identification of high needs patients who, due to financial, social and other barriers, do not currently access the services offered by the 24 Hour Surgery at a level consistent with optimal health outcomes.

The initiative supports all high needs patients who are:
  • enrolled with a Partnership Health Canterbury affiliated general practice, have a history of frequent attendances at the Emergency Department or their practice (but are relatively infrequent presenters at the 24 Hour Surgery), and are prioritised according to age, ethnicity, health status, NZ Dep (New Zealand index of deprivation); or
  • those who are identified by their General Practice (GP) team, Emergency Department or Community Support Nurse as high needs patients for whom a financial barrier is perceived to exist preventing access to after hours care.

The goal is to improve access after hours care by:
  • proactively linking patients to their GP’s preferred after hours provider;
  • reducing the cost of access;
  • encouraging/strengthening linkages between high needs patients and their GP team; and
  • improving patients’ understanding of what is available and when they should access after hours care.

Project feedback was positive in the main, but objections were raised by some on the following grounds:
  • Those practices that provide their own extended hours services felt the project disadvantaged them and their patients. Funding has since been extended to cover these practices for their enrolled patients.
  • Some commented that the project had used a very ‘blunt tool’ for targeting those in greatest need.
  • Some felt that fees charged to patients attending After Hours was so low it may encourage them to wait and utilise this service instead of appropriately accessing their GP team.

Project / Service DescriptionLed by the Christchurch City Council, Active Canterbury is a collaborative project that aims to give physical activity providers, health professionals (including General Practitioners [GPs] and Practice Nurses) and the general public the tools and information needed for more people in Canterbury to be more active, more often.

The strategies were developed after surveys undertaken by SPARC (Sport and Recreation New Zealand) showed that people have a high degree of trust in the information they receive from medical professionals.

The 2006-07 year saw the launch of www.activecanterbury.org.nz, a website which is a one-stop shop of information on how to get active in Canterbury. Also launched were a number of tools for physical activity providers including an easy evaluation tool and an easy promotion tool.

Project / Service DescriptionLaunched in April 2005, the Active Families Pilot project was run in collaboration with Sport Canterbury. It provides resources and support to the general practice teams for children with obesity and their families.

Evaluation of the project was undertaken. While there were identified changes occurring, they did not warrant further investment in this delivery model and the project was not continued. Other models are being explored.

Project / Service DescriptionThe term ‘Acute Demand Management’ describes a cluster of processes that are put in place across a health service to reduce demand on acute secondary care services. This is presently one of nine project streams, under the umbrella of Acute Demand & After Hours Care, aimed at providing services across the Canterbury region to support the management in the community of people presenting in an acute state.

Implementation of five service components commenced in October 2007:
  • Service coordination: Coordinate referrals of people needing acute community based support services in Canterbury.
  • Packages of care: Refers to the range of services that are placed around a person to in an acute state to be managed in the community. The expectation is that these packages will treat the presenting symptoms and prevent or delay a hospital admission. These packages are aimed at supporting the client to remain in their home, and will support clinical involvement from relevant providers.
  • Community observation: This service offers community based observation and/or diagnosis, and symptom control and treatment, as a safe, patient-acceptable alternative to hospital admission for people with acute and sub-acute conditions.
  • Acute community nursing: Describes a range of responsive, flexible and skilled nursing interventions in a patient’s own home or other community setting, with appropriate clinical back-up, to facilitate the process of Acute Demand Management. The expectation is that this service will treat the presenting symptoms and offer a safe, patient-acceptable alternative to hospital admission.
  • Rapid diagnostics: Offers selected diagnostic tests reported within a maximum of 4 hours of referral from a General Practitioner, to support the care of people who may otherwise require hospital assessment and/or admission and for whom provision of care in the community setting will not further compromise their health status.

Project / Service DescriptionTe Tai Tokerau PHO (TTTPHO) has established three integrated nurse-led adolescent health clinics, located in proximity to three schools, two of which are high schools and one an area school, however all of which are in areas of high deprivation (i.e., Kawakawa, Taipa and Kaitaia).

The project has resourced / resources:
  • the establishment and ongoing maintenance of these three clinics;
  • three nurse coordinators to a total of 2.6 FTE (full time equivalent);
  • medical clinics one day per week at each site;
  • ongoing case management support to all sites (by a TTTPHO nursing integration leader); and
  • support for professional development for all those involved in providing this service.

Each of these clinics incorporates services provided by a range of other providers (i.e., public health, sexual health, counselling, Maori health, health providing schools) and there is a memorandum of understanding (MOU) between all of these participating organisations.

A challenge has been encouraging true integration between all of these providers, all of whom have contracts to provide ‘bits’ of services. Another challenge has been aligning policies and procedures of the clinics with the requirements of the Education Service. An Advisory Committee meets once every six months to review progress.

Project / Service DescriptionPurpose: To provide easy access to a free culturally appropriate, youth friendly school-based health service for young people who do not access healthcare elsewhere.

The key components of this service are:

  • To support the development of multi disciplinary school heath teams in all participating schools – currently all secondary schools, the Alternative Education schools and a Teen Mothers unit participate in this project.
  • To lead and/or participate in specific projects identified as priority areas by the school health teams – examples include no smoking initiatives, working with students to address the quality of food in the school canteen, gaining feedback from students about perceived gaps in service delivery.
  • To facilitate the provision of medical and other health related services to students within the secondary environment.
This service is managed by a co-ordinator employed by Manaia PHO.

Project / Service DescriptionAppetite For Life (AFL) is 6-week-long healthy living programme for women delivered through general practices.

The programme covers nutrition issues as well as social and environmental influences on food choices, with food tasting to reinforce the theme for each session.

This project is aimed at supporting all Canterbury PHOs to deliver AFL in the practice setting and in appropriate communities through:
  • training of practice staff to become approved AFL facilitators;
  • management of network of approved AFL facilitators to support delivery;
  • an ongoing support mechanism for the facilitators;
  • an online resource for sharing resources, updating information and a database of approved AFL facilitators; and
  • development of a framework for expansion of AFL for Canterbury.

Project / Service DescriptionPartnership Health Canterbury is researching the Primary Health Care needs of the Asian population in a Canterbury-wide project to understand their social, educational and health needs. The object of the project is to ascertain the problems and suggest possible resolutions in the area of primary health care needs.

With the increasing Asian population in Canterbury, we regard this project as vital to increasing our knowledge and reducing health discrepancies. We cannot achieve these aims unless we enlist the help and input of Asian peoples -- both newly arrived and long-term residents.

The project involves a questionnaire, made available in English as well as multiple Asian languages. All received questionnaires are confidential with the results being collated as part of the final report, which will be presented to the Canterbury District Health Board for action.

Project / Service DescriptionPartnership Health commissioned Sport Canterbury to research the barriers for young females (aged between 15 and 18) to exercise. The research involved 11 different schools and other organisations.

Researchers found the main barrier to being physically active is a lack of free time, while the main motivator is being active with someone else, in a social setting.

The survey recommended more 'out of school' activities be made available to the girls to get them active. This could include low-cost memberships to gyms, pools or leisure complexes. To counter the barrier of a lack of free time, physical activities need to be built into everyday life.

Project / Service DescriptionThis funding is to support a targeted cardiovascular risk assessment and management pilot programme in 5 general practice settings and two large industrial sites. The pilot is for a 2 year period and includes the set-up in practices/industry at staged times, initial risk assessment/management of individuals identified as high risk, regular telephone follow-up and re-risk screen at one year for those individuals who CVD risk 15-20% or >2%

Project / Service DescriptionCarePlus is a programme that provides support and coordination of care for people with chronic diseases or high health needs. CarePlus aims to improve quality of life, reduce hospitalisations, improve management of chronic conditions, reduce inequalities, facilitate teamwork within PHOs, and increase access for high-need primary health users. The core functions of CarePlus are to ensure that care for patients with high health needs is better coordinated and to provide some funding to support access to additional services that will improve their health.

As at 30 June 2007, 8,034 people in Canterbury were enrolled in CarePlus, more than double the figure for the 2005/06 year. Of these, 6,465 are European, 574 are Maori, 187 are Asian and 223 identified themselves as Pacific people. These figures exceeded our targets for the 2006/07 year.

Comprehensive progress is also being made on developing a software package to simplify the registration and claims process for practices. This will encourage practices to continue to enrol patients into CarePlus.

A survey done in 2005 found that 91% of patients felt their medical care was better as a result of CarePlus, 86% of people were satisfied with the benefits of the CarePlus programme, and 59% of people saw a doctor more than they usually would as a result of being on the CarePlus programme.

General Practices have also been surveyed for feedback on CarePlus. The key findings of that survey were that 90% of general practices surveyed agreed that CarePlus is beneficial to patients who need extra care and 82% of respondents said they would recommend CarePlus to other practices.

The service employs ChildLink workers, whose key aims include the provision of information about services available and the enhancement of communication and liaison between general practice teams and existing child-focused service providers.

The team receives referrals from health care and community agencies, as well as individuals, when there are concerns for a child's health or wellbeing. They can then work with existing services to address these concerns.

ChildLink is assisting children and their families and whanau to receive the assistance, treatment and support they need. In the process, it is helping to identify gaps, encouraging collaboration, and improving the links and information flows between service providers.

Project / Service DescriptionIn 2006/07 Partnership Health Canterbury piloted the Chronic Condition Management (CCM) programme at 10 general practices across Canterbury.

The programme is designed to provide packages of care for individuals who are newly diagnosed with a chronic condition or for those who have recently been discharged from hospital.

It works by providing funding to general practices to enable them to spend additional time with the patient to create a management plan for their care. In addition, the programme employs a coordinator who identifies service providers, educates general practice as to what services are on offer and ensures appointments are available for those enrolled in the CCM programme.

During May and June 2007 (the first two months of the pilot), 53 patients were referred to the CCM programme; 46 of which had been newly diagnosed with a chronic ongoing illness.

Project / Service DescriptionCATINC (Community Action to Improve Nutritional Capacity) is a 30-month-long project, which focuses on creating environments supportive of healthy nutrition with easy access to healthy food. Partnership Health is leading this project, working alongside all the other PHOs in Canterbury, the Canterbury District Health Board and the Ministry of Health.

The project employs two full-time staff members whose job is to work with communities most in need of nutrition education and enabling them to make healthy food choices.

The initiatives which will be undertaken will differ between communities, but may include:
  • increasing knowledge in general practices on good nutrition;
  • educating church and community groups on healthy foods;
  • ensuring vending machines are sugar free;
  • improving access to fresh fruit and vegetables; and
  • eliminating junk food advertising from communities.

The cooperation of all the Canterbury PHOs on this project means CATINC is being implemented over the wider region, from Kaikoura to Ashburton.

Project / Service DescriptionThis service is aimed at improving the appropriate utilisation and access to health care for high needs patients with an identified chronic condition.

The identified chronic conditions include:
  • PHO Expert Patient - The programme seeks to identify and train suitable people with Chronic Conditions to deliver the Expert Patient Programme. This will increase the delivery capacity of chronic care programmes.
  • Chronic Obstructive Pulmonary Disease (COPD) - To ensure that patients with COPD have demonstrable, consistent access to diagnostics and assessment, with long term and acute management delivered to best practice level in both hospital and community settings by collaborative staff and resources.
  • Community Support Nurse - To provide case co-ordination as required to complement and to support the involvement of other healthcare professionals.
  • Cardiovascular Disease (CVD) - To reduce the incidence and impact of CVD through early detection and intervention for those most at risk.

This is an 18-month one-off pilot funding to establish, develop and maintain a programme aimed at people with a history of high utilisation of health services, due to one or more existing chronic conditions. This service is currently in the planning stages and will particularly focus on people with a high number of Emergency Department attendances.

Project / Service DescriptionThe End-of-Life Primary Care Support Care Service will provide support, education and back up for General Practices to enable them to continue to provide end-of-life care to all enrolled people within Partnership Health Canterbury who are eligible for and need the service.

The Service will include:
  • A special situation benefit – To assist patients who wish to remain under the care of their General Practitioner during the end stage of their cancer-related illness by subsidising the cost of their primary medical fees.
  • Palliative care services - This is a liaison service which enables part-time specialised General Practitioners to offer advice, support and assistance to primary care practitioners caring for terminally ill patients. The specialised General Practitioners will liaise with other services in Christchurch providing care for terminally ill patients.

Project / Service DescriptionA Sport and Recreation New Zealand initiative, Green Prescription (GRx) is an exercise referral progamme which helps individuals build regular, health-restoring activity into their day-to-day lifestyles.

More than 15,000 people have been referred to the programme through their general practice nationally. Each person is given phone support and advice over four months. GRx Community Clubs are also established in some locations. People with heart disease, diabetes, obesity, mental health and arthritic conditions can gain huge benefits by increasing their physical activity levels. Sedentary people should be referred to GRx, to prevent future health risks.

Partnership Health Canterbury provided additional support to Sport Canterbury to increase the use of GRx for our PHO. The programme was successful with almost a 100% increase in referrals during the first half of the project.

Project / Service DescriptionPhysical activity and nutrition are major health concerns as they underline many of the chronic conditions faced by individuals and communities and place huge demands on secondary services.

The Flamin' Healthy Campaign was a core achievement in promoting physical activity and nutrition. Partnership Health teamed up with the Goldair Canterbury Flames netball team and Fresh Choice supermarkets for this campaign.

We sourced healthy recipes with Community and Public Health and made them available in the supermarkets, the Flames players were promoted as 'healthy eating heroes' and cooking demonstrations were held in-store.

The campaign received excellent feedback with more than 82,000 recipes distributed through the supermarkets.

Project / Service DescriptionThis initiative assists General Practice in caring for their frail elderly patients in the community. It is designed to help these patients access the right care in the right place without the financial burden that might otherwise reduce their access to care.

It is for:
  • People who are frail and “only just” managing independently in the community.
  • Patients who are becoming frail, and are likely to require extensive care. This is often the result of an illness or minor crisis, where frequent visits would be required.
  • Vulnerable elderly people who have struggled at home and may have a combination of acute on chronic problems which are threatening to tip them over into residential care/hospital admission.

Following a successful pilot the project has been rolled out to all Partnership Health practices. As of 24 August 2007, 172 registrations have been received from 60 General Practitioners.

Project / Service DescriptionThis project was approved by the Board in August 2007 to support gazetted General Practitioners (GPs) to maintain a high quality service for patients on methadone, keeping both patients and GPs safe. The initiative entails subsidised GP visits, thus reducing barriers to health.

Support includes a committee comprising of 3 GPs, the Pegasus Clinical Leader of Mental Health, 2 representatives from the Christchurch Methadone Programme, a psychiatrist and a consumer representative.

Since 2004, patient numbers in the programme have gone from 35 to 117, with an additional 18 patients successfully counted off methadone with no relapse to date.

The impacts of the programme extend into wider society as lives are rendered more functional, with impacts such reduced spread of infection from sharing of IV needles and reduced criminal activity.

It is proposed that future funding recognition of the programme by the Canterbury District Health Board is sought.

Project / Service DescriptionA key achievement for Partnership Health is the establishment of a Community Grants scheme. This scheme enables individual community groups to apply for funding to enable them to get initiatives which will improve primary health already underway.

To date Partnership Health has funded over $60,000 to community groups and organisations working to improve the primary health of our population.

The projects supported include:
  • Tangata 2 Tangata – Triathlon targeting Maori and Pacific peoples.
  • Mental Health Education and Resource Centre - Used a grant to enable them to supply their library with resources on Nutrition, Physical Activity and the importance of being Smokefree.
  • Aranui Community Café project - To offer healthy food to the community had stalled due to a lack of project management experience. Grant money enabled the establishment of a community group, encouraging physical activity among at-risk youths. Sport Canterbury and The Youth Centre used grant money to run an exercise programme for troubled youths.
  • Maori Sports and Health Festival – Partnership Health Canterbury sponsored this event which celebrated healthy active Maori.
  • Mental Health Awareness Week - Used funding to screen the well-known movie Supersize Me at a Christchurch cinema to highlight how good nutrition can influence mental health.
  • LiveSmart Expo – This expo promoting health and wellness initiatives was held across five locations in the Selwyn district.
  • Lincoln and Districts Community Trust – This activity programme for seniors has had real results for members and the wider community.

Project / Service DescriptionIn 2006, as a step towards being a ‘health-promoting PHO’, Manaia Health set aside funding for health promotion initiatives in general practice. To encourage the development of effective health promotion, an introductory health promotion training programme was created for Manaia Health PHO practice nurses.

In 2006, the training involved eight two-hour evening sessions, which commenced with dinner. Thirteen practice nurses completed the training. In 2007, the training involved three two-hour evenings and one Saturday morning, and the training was also extended to include all primary health care nurses.

The objectives of the programme are:
  1. to provide primary health care nurses with a comprehensive understanding of health promotion models, values, theory and practice;
  2. to assist primary health care nurses to plan, implement and evaluate a health promotion project; and
  3. to build a team of 'health promotion champions' within Manaia Health PHO

To date several health promotion projects have arisen as a result of the training. These include:
  • a ‘life switch clinic’ focusing on supporting people who want to make changes in their life;
  • a men’s health screening project’;
  • a community development project in a local suburb of Whangarei; and
  • a nurse facilitated walking / support group.

Projects in development from the 2007 training include:
  • kaupapa Maori antenatal classes;
  • the development of an elderly support resource for general practice; and
  • smoking cessation clinics.
Annual Budget (Optional)Funding comes from the health promotion budget. $60,000 is set aside for health promotion projects, this includes projects already developed and funding for 2007-08 projects.

The training costs are about $1500, which involves payment to practices for nurse time and catering expenses. We use the PHO meeting room and do not need to pay for external trainers/facilitators. Photocopying/resources costs would need to be added to this budget.

Project / Service DescriptionPartnership Health Canterbury has expanded the National Heart Foundation's Healthy Heart Award programme into early childhood centres in Canterbury.

This is a comprehensive programme which assesses both the nutrition standards and physical activity programmes of individual organisations. Once the centre has passed a total of seven modules, they receive their award.

As of July 2007, Christchurch now holds 30% of the total number of Healthy Heart Awards awarded nationally. Our support has meant that there are now 88 Canterbury early childhood centres enrolled in the scheme. The campaign now has over 5,000 children in these Early Childhood Centres consuming milk and water and eating fruit daily.

  • who have a stable medical condition which may improve with an improvement in physical activity levels and eating patterns; and
  • who are ready to make positive changes with respect to lifestyle choices.

Project / Service DescriptionFree hearing screening for kaumatua attending a rural Marae-based Kaumatua programme delivered by Te Hauora o Turanganui a Kiwa (Turanga Health), to identify kaumatua with a hearing deficient, who were referred on for further assessment either to their respective general practice for referral to ACC, or to the local audiologist. Of those 200 screened, 36 kaumatua were ineligible for ACC funded hearing aids.

Project / Service DescriptionThe Rodger Wright Centre, Hepatitis C Resource Centre, and Needle Exchange New Zealand all have a vested interest in injecting drug user (IDU) health and hepatitis C prevention and management, so they proposed the inception of a hepatitis C community clinic.

The clinic is running as a pilot project based at the Rodger Wright Centre, and offers a point of entry to free comprehensive hepatitis C identification and management. It aims to reduce barriers to IDU health care, improve hepatitis C health outcomes and optimise access to treatment (within a shared-care framework with General Practitioners and Canterbury District Health Board treatment providers) for the targeted population.

Project / Service DescriptionThis is a 24-month-long project is funded by Partnership Health PHO and run by Smokefree Canterbury (Partnership Health Canterbury currently chairs this group). It focuses on Hornby; a low decile area on the outskirts of Christchurch with a high Maori and Pacific population.

The project aims to promote the Smokefree cars and homes initiatives, by encouraging smokers to quit, reducing the uptake of smoking and promoting internal and external smokefree environments.

The project employs a half-time community worker who trains and resources community groups to promote smokefree messages, connects them (and individuals) with smoking cessation services, facilitates delivery of workplace smoking cessation programmes (where appropriate), and develops and initiates opportunities to promote smokefree environments.

One of the Hornby project's achievements in the 2006/07 year was to support a group of Hornby High School students to be proactive in addressing youth smoking issues. The students presented at the Riccarton Wigram Community Board and requested support in making parks near the school smokefree. This was successful and the result is a Hornby Smokefree Playgrounds 3-month pilot programme scheduled to begin in April 2008.

Project / Service DescriptionDue to the rapid increase in patients for whom English is not their primary language, Language Line was introduced as a free service for both practice staff and patients. Using the Department of Ethnic Affairs Language Line, all practice staff have access to qualified interpreters most of whom have a medical background. This will reduce or eliminate the possibility of medical and cultural misunderstandings.

Project / Service DescriptionInvesting in Health Outcomes is FamilyCare PHO’s three-year Strategic Plan. Phase One is the development and implementation of its Chronic Care Management (CCM) Programme which has been developed to ensure that greater health outcomes for its enrolled patients are realised.

Implementation of Phase One is to commence with the development and creation of a “Whole of System” approach to primary healthcare for those patients who suffer chronic disease. This first phase has been developed using Predict as the vehicle for which FamilyCare’s CCM programme is based. Health Action/ Management Plans will be created from existing CarePlus Plans.

The service mix will entail utilising:
  • outreach nursing and doctor visits/clinics;
  • health promotion activity; and
  • horizontal analyses of a patients needs based upon a “from birth to grave” perspective.

Targeting of services will focus at Maori, Pacific and Q5 population groups with the development of Outreach Nurse and Doctor Clinics.

Please note that this strategy has yet to have Board sign-off. Anticipated that the Board accept the concept at its meeting of 23 July 2007 and ratified at AGM in September 2007.
Annual Budget (Optional)Funding from Management and PHO Performance Management will be utilised to ensure the Continuous Quality Improvement (CQI) principles to patient care are adhered to. Anticipated first up costs $110,000 - $200,000, and sustainable funding modelled against projected cash flows for 2008-09.

Project / Service DescriptionThe project resources the appointment of two FTE (full time equivalent) registered nurses (Maori) with advanced skills in cardiac nursing and two 0.6 enrolled nurses who support them, to work with Maori to reduce the impact of cardiovascular disease across the care continuum.

The nurses work under the umbrella of two key stakeholders of Te Tai Tokerau PHO (TTTPHO), Ngati Hine Health Trust and Te Hauora o Te Hiku o Te Ika, and are located in the mid North (Kawakawa) and the far north (Kaitaia).
  • 4 FTE (full time equivalent) of this role is to lead cardiovascular screening in outreach situations across their relevant areas using PREDICT, and subsequent follow up with practices with whom the patients with high risk identified are enrolled.
  • 0.6 FTE (full time equivalent) of this role is dedicated to providing Maori who have had a cardiac event with kaupapa Maori cardiac rehabilitation programmes. This programme is based on the NZ evidenced based cardiac rehabilitation guideline, that has been put into a wider kaupapa Maori framework.
  • The annual resource allocation covers employment, overheads, travel, equipment, programme development, professional development and mentoring, etc.

The project is being evaluated by Whaariki (associated with Massey University).

Project / Service DescriptionMaori women smoke at almost twice the rate (51.9 percent) of Pakeha women (20.7 percent).

In the 2005-06, Partnership Health supported the Aukati Kai Paipa programme run by Hauora Matauraka which actively targets Maori women, focusing on supporting them to cease smoking. The programme is intensive and is shown to have an average quit rate of 30 percent.

The primary aim of the programme was to create a linkage between mainstream general practice and Aukati Kai Paipa, a Maori-for-Maori cessation programme. Partnership Health’s involvement enabled the programme to expand with the goal of supporting an additional 100 whanau.

Project / Service DescriptionThe six Northland PHOs were successful in securing Primary Mental Health Care Initiatives Funding for a regional strategy that includes the following objectives:

  • To improve the identification, treatment and support for people with mental health and addiction issues in Northland.
  • To provide better follow up and coordination for people with mental health and addiction issues in Northland
  • To promote better understanding and prevention of mental health and addiction issues in Northland.
This proposal promotes and integrated regional model for Northland that consists of:

  • The development of new practitioner role/s that focus on the care of people with mild-moderate mental health issues
  • Development of a relapse prevention service for people living with long-term mental health problems who can be managed in the Primary Health Care setting
  • Ensuring a well prepared Primary Health Care workforce
  • The development of a tailored information technological package
  • Subsidised counselling
  • Therapeutic recreation.
All components except for therapeutic recreation impact upon Manaia Health PHO.

Manaia PHO employs the mental health co-ordinators and an administrative support role.

Project / Service DescriptionPai Ake is a kaupapa Maori men’s service which specialises in delivering men’s mental health and addiction services. This initiative will outreach two existing successful programmes as well as identifying potential new train the trainers in rural Waikato. The programmes were developed for men and youth who do not have a history of successfully engaging with services, and who have mental health and addiction issues relating to low self esteem and sense of identity.

The evaluation of the programme and outcomes is being conducted by Waikato University.

Project / Service DescriptionThis service was to enable and support the General Practitioner (GP) to provide palliative care services to those patients who are at the end stage of a terminal disease. The home visits will be provided by the patient’s own GP (or the GP on call – as this service is available in rural communities, close communication could ensure an on-call GP is aware of all terminal patients in the area). Timely access and early referral to these services is effective in improving the quality of life for people dying and undoubtedly reduces the stress and worry of the surrounding family.

Project / Service DescriptionThe risks involved in smoking are well known but even so, there are still nearly 5,000 tobacco related deaths in New Zealand every year. That is more than all the fatalities caused by road crashes, suicide, skin cancers, drowning, homicide and AIDS combined.

The original PEGS Smoking Cessation Programme resources have been updated and additional resources, including pamphlets and posters, have been created. These resources, which are now also available on the Pegasus Health intranet, are used to support delivery of the PEGS programme by all of the general practices within Partnership Health.

Regular PEGS workshops provide training for new practice staff and information updates and phone support ensure that practice staff are informed about recent developments in cessation, and any specific questions they have are answered promptly. Effort is directed at creating and seizing opportunities to promote PEGS to the general practice teams.

More than 2,360 people enrolled in the programme during the 2006/07 year. Of these 55% were female and 9% were Maori or Pacific peoples. PEGS' average quit rate is between 23% and 36%, which compares well with other programmes.

Project / Service DescriptionThe Physical Activity and Nutrition (PAN) Service Map was created in April 2005 as a collaborative project funded by Partnership Health Canterbury (Primary Health Organisation), and Community and Public Health (Canterbury District Health Board), that has attempted to document the activities undertaken by a variety of organisations.

The Map researched and identified the groups and organisations involved in projects and programmes to encourage and support people in being active and eating well.

Because of the large range of activities and services that impact on physical activity and nutrition in Canterbury, medical treatment providers, commercial ventures, sporting organisations (such as the Netball Association), and programmes within which physical activity or nutrition was a minor component were excluded from this mapping activity, leaving the focus on health promotion initiatives.

Information from identified organisations and their associated projects, programmes and initiatives was gathered using a questionnaire and face-to-face interviews. Key themes were identified from the data.

The Service Map identified 43 projects, programmes and initiatives which promote nutrition and physical activity in Canterbury. It also found there was room for improving the co-ordination and integration between the projects and that evaluation of these services needs to be developed.

A final report was published in April 2005.

Project / Service DescriptionThrough this initiative two FTE (full time equivalent) podiatrists are employed to provide primary podiatry and related orthotics services to patients in high need areas. Consultations are fully funded, and the podiatrist travel to allow easy access to satellite clinics, so part of this project has involved the purchase of equipment to enable services to be taken to outlying areas. Patients are predominantly those with diabetes but include a range of other morbidities as well. The service was initiated because we identified a huge gap in the podiatry services provided by the provider arm of the DHB, and interestingly these have diminished further since the SIA project was initiated. The podiatrists have developed clear referral criteria, and have provided training to practice staff to raise the level of confidence of nursing staff in particular to manage the “uncomplicated foot” and to provide a comprehensive assessment of patients having a diabetes annual review.

Other topics were discussed in response to specific questions from the practice.

This project informed PHO communication strategies, and resulted in increased PEGS enrolments and increased referrals to Green Prescription.

Project / Service DescriptionThis suite of projects contributes to a Workforce Development Plan to be developed and implemented over the period of July 2006 to July 2008.

The programme objectives are to:
  • to build capacity and capability within the general practice workforce;
  • to develop and implement new initiatives to prepare the workforce for the broad clinical responsibilities of delivering high quality care in a changing environment; and
  • to provide an effective and comprehensive solution for enhanced delivery of workplace initiatives within Partnership Health Canterbury.

Service Components include:
  • Introduction to Primary Care Nursing courses;
  • orientation tools, resources and support for new nurses;
  • Return to Nursing programme support;
  • supporting clinical placements for medical and nursing students;
  • General Practice Scholarships for medical trainees;
  • exploration of an agency role;
  • New Entry to Nursing Practice programmes; and
  • Future Primary Care Team development.

Progress has been made in all of these components. However it has proven easier to make a difference in the nursing sector within the current funding levels than in the medical workforce.

A further proposal is currently being developed to seek a further injection of funding to specifically target general practice medical workforce issues. We have an aging General Practitioner workforce with few new entrants. This is of significant concern to the sustainability of a quality primary care service in the future.

Project / Service DescriptionThe purpose of this position is to provide a comprehensive and culturally appropriate clinical dietetic service to meet the needs of patients enrolled with Manaia PHO. The position works 0.5 with patients referred from general practice, and 0.5 with Maori health providers, Hauora Whanui (HW) and Kia Ora Ngatiwai (KANW). The dietitian undertakes individual case management, facilitates groups where appropriate, provides education and support for general practice staff and staff employed by the Maori providers, and is involved in a range of health promotion activities. In order to manage the increasing workload, a second dietitian has recently been employed.

Both dietitians are employed by Manaia Health PHO.

Project / Service DescriptionProject One is the principal project within the suite of Partnership Health Canterbury’s “Services to Improve Access” initiatives aiming to improve access to healthcare services for Maori, Pacific Peoples and lower socio-income sector.

Project One enables general practice teams and community agencies to identify barriers to a patient/family/whanau in accessing health care (e.g. financial, transport, language, cultural) and to have access to funding to address these barriers, in effect creating “pathways” to access general practice with three supporting components – Flexible Funding, Practice Nurse Intervention funding, and Partnership Community Workers supporting general practice teams.

Approximately 50 percent of practices within the PHO are now accessing this initiative. Three-monthly patient outcome reports are provided by each surgery and these reports indicate appropriate use and innovative care being provided utilizing the flexible funding package, including significant linking with other providers and services.

Practice Nurses work in collaboration with appointed Partnership Community Workers to liaise with their local communities targeting the high-risk, disadvantaged population to assist with improving access to ongoing, appropriate primary health care services.

Partnership Community Workers are employed by their umbrella organisations and subcontracted to the PHO and work in collaboration with a designated group of general practice teams and local communities to identify any at-risk people, and as appropriate refer to general practice. They also accept referrals from general practice to further assist the ongoing care of SIA patients (e.g., working with WINZ [Work and Income New Zealand]).

Project / Service DescriptionProviding Access To Health Solutions (PATHS) is a programme that coordinates health and employment services for people who are receiving a Sickness or Invalids’ Benefit and want to return to work.

This service is provided in Canterbury by Partnership Health Canterbury, Work and Income, and the Mental Health Education and Resource Centre (MHERC), and is funded by the Ministry of Social Development (MSD).

PATHS employs its own Health Coordinator (provided by Pegasus Health), who works closely with a client’s General Practice to look at ways of improving the client’s health and assisting him or her to reach the goal of returning to work.

This process may involve linking the person with community support groups, counselling and specialist assessments; it could even include arranging surgery. PATHS also refers clients to Work and Income, Work Brokers and Supported Employment Agencies, which can assist them in finding employment that is sustainable for them.

For the 2006/07 year PATHS had 75 new enrolments; 64 people progressed into either part- or full-time employment or are actively supported by an employment service.

Project / Service DescriptionThe Quit to Win (QTW) Smoking Cessation Incentive Programme was about rewarding smokers – if they quit. The programme was run in conjunction with the PEGS (Preparation, Education, Giving Up and Staying Smokefree) and Aukati Kai Paipa programmes.

People enrolled in the PHO who are part of an eligible smoking cessation programme are automatically entered into a monthly prize draw. Winners who are verified as smoke-free by their General Practice Team, receive prizes donated by Canterbury businesses like movie tickets, meal vouchers, gym memberships, spa and massage packages and haircuts. Over 400 prizes were given out during the programme, This was supported by a media campaign to increase awareness.

Project / Service DescriptionMeeting the primary healthcare needs of refugees to New Zealand is both a complex and rewarding task for General Practice. Refugees lack knowledge of how to actually access primary healthcare in New Zealand. This, coupled with language and cultural differences, transport difficulties, trust issues and trauma-related concerns, makes for time-consuming health work.

During 2006-07, Partnership Health Canterbury contracted the Christchurch Resettlement Centre to undertake a four-month long research project on refugee health needs. The results have allowed us to more accurately target our funding.

We can now offer additional funding to those General Practices that have 50 or more enrolled refugees through a new service: ‘Refugee Primary Healthcare’. This funding is making a real difference to the lives of refugees, not only by subsidising health services, including General Practice time, mammograms and immunisations, but also by covering other costs, such as interpreters, x-rays and transport.

Overall this project is helping to remove many of the barriers refugees face in accessing primary healthcare.

Project / Service DescriptionFree sexual health consultations with a General Practitioner or practice nurse have been available for under-25-year-olds since March 2006, and is one of three Sexual Health SIA options provided in Turanganui a Kiwa. The aim of the free service is to reduce the number of unwanted pregnancies and reduce the chlamydia infection rate. An estimated 5723 enrolled patients are eligible for the free sexual health service.

As of December 2006, 667 people, mainly women (94%), had taken advantage of the service. The breakdown of ethnic groups that have utilized the service shows 57% were European, 41% were Maori, 1% were Pacific, and 1% were other.

Project / Service DescriptionThe Canterbury Smokefree Initiatives Service Map was created in April 2005 as a collaborative project funded by Partnership Health (Canterbury Primary Health Organisation), and Community and Public Health (Canterbury District Health Board).

The purpose of the service mapping exercise was to perform a stocktake of current smokefree initiatives in Canterbury to act as a foundation for future planning and delivery of services and other relevant activities. While many smokefree health promotion and smoking cessation activities were being conducted in Canterbury, greater coordination and strategically applied funding would improve the quality of these services.

A questionnaire was developed to collect information about the organisations involved, their smokefree health promotion projects and/or smoking cessation programmes. The questionnaire was administered using a semi-structured face-to-face interview. Responses were entered into a database and information thematically coded, then analysed.

Based on their findings from this research, the authors have made recommendations to improve future planning and delivery of smokefree services in Canterbury. The final report was published in April 2005.

Project / Service DescriptionVarious models of delivering Smoking Cessation in workplaces have been piloted:
  • a Practice Nurse delivering PEGS (Preparation, Education, Giving Up and Staying Smokefree) in local workplaces;
  • a General Practitioner delivering PEGS in a workplace where he also delivered other GP services;
  • a contracted nurse with addictions training delivered PEGS and additional group sessions in a workplace; and
  • a contracted nurse delivered PEGS with some additional sessions to a group which came from a variety of workplaces in the same geographical location.

Abstinence rates at the end of the programme were high however, there has been some difficulty ascertaining the 6 month quit rate.

To date, the GP delivered pilot was partially funded by the workplace and the other pilots were fully funded. Further pilot programmes based on a co-funded model (workplace and PHO) are needed to assess the sustainability of this programme.

Project / Service DescriptionThis initiative entails several pilot programmes at Canterbury hospitals targeting people who signal a wish to give up smoking while in hospital. The programme ensures that they receive the support needed to cease smoking once they are back in the community.

Patients are given free Nicotine Replacement Therapy when they are discharged and are also referred to their general practice team, which offers additional support, including the opportunity to enrol in the PEGS (Preparation, Education, Giving Up and Staying Smokefree) programme.

Nearly 17 percent of those who enrolled in the pilots went on to enrol in PEGS.

Other PHOs and hospitals from outside Canterbury have shown interest in the resources and framework which are available from the PHO.

Project / Service DescriptionEmployment of a social worker who will, in collaboration with the general practice team develop and provide a comprehensive and culturally appropriate medical social work service to meet the needs of patients enrolled with Paramount Medical Centre.

The key purpose of the role is to work with individuals, families and Whanau in order to support the management of illness and / or disability and associated issues. The aim is to provide professional and effective social work assessment, planning and intervention/s within the context of a multi-disciplinary team to restore individual, family/Whanau, community well-being. Enhancing the development of client self determination, and promoting principles of social justice are core aspects if the role. As the position becomes more established, the social worker will also potentially broaden the role to encompass aspects such as working with others to build community capacity, participating in relevant legislative and social processes to improve health and social services, and educating others on the impact of social and environmental factors in health and well-being.

Project / Service DescriptionMore than 125 individuals from 21 Tongan families in Christchurch signed up to this pilot community action initiative, which was run by the National Heart Foundation with Partnership Health Canterbury funding.

This is an education-based programme, with those taking part attending 13 information sessions at the Tongan Community Association. The sessions involved a shared meal and covered topics including health, nutrition and physical activity.

At the beginning of the programme, each individual’s health was assessed. Seven people were found to be at risk of diabetes and seven who were at risk of cardiovascular disease. These results enabled the facilitators to ensure those individuals received appropriate medical intervention.

Three months after the programme ended, a follow-up meeting was held to see what differences had been made. Successes included:
  • an obese man who had lost 25 kilograms;
  • changes to shopping habits, with a number of mothers declaring their homes ‘junk-food free’;
  • awareness of healthy alternatives - one demonstration involved separating and cooling the fat from a heated can of corned beef which proved especially powerful with many of the families now using healthier alternatives to this popular food;
  • changes of attitude - many reported they were more aware of portion size, with one father saying his family is learning “not everything needs to be eaten at once”; and
  • greater interest in physical activity - an exercise class has now been set up at the Tongan Community Centre and is well attended.

Project / Service DescriptionThe key objective of this service development is to improve health outcomes by removing barriers and enhancing access to healthcare services for the people of Otangarei. This service focuses on enhancing health services delivered through the established community owned primary health care centre in the area.

The service involves the establishment of three new positions, namely a primary health care nurse (1.0 Full Time Equivalent (FTE)), a community health worker (0.5 FTE), and a health promotion position (1.0 FTE). A key focus for the nursing role is to undertake comprehensive health assessments/development of on-going plans for patients who present opportunistically to the health centre, and to undertake a care co-ordination role for patients with complex health and social needs. The CHW provides an important linkage with the community and will utilise multiple strategies to enhance access. The health promotion role provides focused health promotion activities that support and enhance service delivery.

This contract is managed by Te Puawaitanga O Otangarei.

Project / Service DescriptionLinking five asthma nurses and two clerical staff from three organisations (Asthma Society, and two Maori Health Providers) by using a remotely accessed Medtech32 system in Kaitaia, Kaikohe, Dargaville and Whangarei. It is a standard MT32 application with advanced forms built to capture workflow/contract info, and validated QOL Asthma surveys (in development). Application sits on Kaipara Care server, and access is via broadband VPN.

Purpose was to:
  • improve the tools available to the nurses who already have training linkages;
  • capture information which supports the value of their work (e.g., Quality of Life surveys);
  • improve the feedback to General Practitioners (inbuilt reports and letters sent to General Practitioners via healthlink and becoming part of the permanent primary care record);
  • centralise reporting on contract requirements; and
  • assist planning for more specialist nurses.

MT32 Training is both in person and via web/phone-conferencing.

Issues include:
  • not enough FTEs (full time equivalents) to cover Northland; and
  • poor broadband quality in Whangarei disrupts the VPN connections frequently.

Project / Service DescriptionThe Whanau Link Project was successfully piloted by Pegasus Health for the twelve month period up to July 2007 and has since been funded through Services to Improve Access and extended to Partnership Health practices.

The service aims to:
  • reduce inequity of health outcomes for Maori;
  • improve access to primary care and effectiveness of service; and
  • increase the perceived need for and the value of general practice to Maori.

The service is delivered by a Whanau Link Coordinator who works with general practice teams and the Maori community to target and support the care of Maori, both with those who are enrolled with Partnership Health practices but not accessing care appropriately, and those who are currently not enrolled.

Access to general practice is enhanced and practical support and resources provided for practices to understand and overcome barriers to access for this group.

Project / Service DescriptionHealth promotion can prevent illness, lower disease rates and limit the pressure on both primary and secondary healthcare services. It is a core focus for Partnership Health Canterbury.

In the 2006-07 year, we supported the Winter Warmth Programme, a collaborative project lead by Community Energy Action and Age Concern with extra funding to ensure it can upgrade the heating and insulation of elderly people's homes in Canterbury.

This has developed into a long term strategic relationship with Energy Efficiency and Conservation Authority (EECA) and local healthy housing partners.

Project / Service DescriptionPartnership Health Canterbury has established a Community Grants scheme, enabling individual community groups to apply for funding for initiatives that will improve primary health. One of the groups to receive funding was the Young Parents’ Breastfeeding Group.

This group was established to provide twice monthly meetings for expectant and new parents aged up to 24 years. The meetings enable those attending to support one another and to access a breastfeeding support worker who offers advice and answers questions.

Project received funding from the HEHA (Healthy Eating Healthy Action) funding and will be supported on an ongoing basis through a specific DHB breastfeeding service programme.
