Capitation Funding A model for funding primary healthcare whereby the enrolled population is funded on a per head basis rather than on a per visit basis
CBFCapitation Based Funding, both an information technology system and a supporting process for funding PHOs on a capitated basis
CCPSClient Claims Processing System, a HealthPAC system that processes payments for providers of health and disability services on behalf of the funders of those services
CMSContract Management System, a HealthPAC system with three main functions: reporting; the generation and administration of agreements between funders and contracted providers of health and disability services; and processing payments to the providers of contracted health and disability services on behalf of the funders of those services
Connected Health CommunityThe collective of health workers and entities (i.e. hospitals, general practitioners, laboratories, pharmacies and other healthcare providers and funders), connectivity service providers and health information / application providers that communicate and collaborate within the Connected Health governance and management framework
