What are Dental Subsidies?
Dental subsidies are available to Dentists who have provided services to children under the age of 18.

Do I need to get my existing patients to fill in the new enrolment forms?
All patients need to complete the new enrolment forms as they come in for their next consultation. If HealthPAC have already received an enrolment form signed by the patient's parents then it is not necessary to have the parents sign the new form.

Where do I send my Adolescent Oral Health Enrolment forms to?

I have run out of Adolescent Oral Health Enrolment forms, where do I get some more?
Enrolment forms are available from Wickliffe Ltd on 0800 259 138 - you will need to provide your payee number.
