What is the Patient Refund Service Payment?
This subsidy is available when a Specialist has opted not to claim on behalf of a patient. The patient pays the full amount and collects the available subsidy directly.

How much can I claim?
The fee payable is $5.45.

Where can I get a form from?
The reimbursement form is provided to the patient by the Specialist.

How do I claim the subsidy?
Your doctor will give you a ‘Claim form for Refund of Patient Subsidy’. Fill in the form, and make sure the doctor signs it. This form is the proof that you have visited the doctor and have been provided with a health service.
The form Includes:

Patients name
Patients Address
Patients Date of Birth
Patients Signature
Patients Details of Bank account
Date of service
Doctor’s name
Doctors Address
Doctor’s signature.
