Care Plus


Care Plus provides additional funding to Primary Health Organisations (PHOs) to provide more comprehensive, co-ordinated care for people who need to visit the doctor more frequently. People eligible for Care Plus include those with two or more long-term conditions, such as diabetes or heart disease; and also those with terminal illness and those who are on active review for elective services.

Many PHOs around the country are offering Care Plus. You could ask your doctor or nurse if your PHO offers it and then, if they do offer it and you have not already been assessed for Care Plus, they may assess you to see if you are eligible. If you are not eligible it may be that they can help you in other ways or review your existing care.
