Evolving Models of Primary Health Care Nursing Practice is a publication highlighting and showcasing eight models of primary health care nursing in New Zealand.
A strong primary health care system relies on a strong workforce. This was implicit in the design and launch of the Primary Health Care Strategy in 2001 and remains just as vital today.
Nurses make up a huge part of that workforce, and it can be argued that their changes in roles and responsibilities are a manifestation of the possibilities of the Strategy itself.

These models are intended as a resource rather than examples of best practice at this stage in the implementation of the Strategy.
The objectives of the project and this resulting publication were to report on:
• how health needs are identified and the populations served
• service initiatives and health care
• new practice developments – the new roles and responsibilities of the Nurse Practitioner
• influencing factors in service and practice development
• future directions of the service and practice.
